BEHAVIORAL HEALTH CARE MANAGER

We are currently looking for a caring, ambitious energetic individual to provide primary and telemedical care for patients transitioning out of prison. We value quality of care, and achieving healthcare metrics, reducing hospitalizations and re-hospitalizations, decreased emergency department utilization. Responsibilities:

  • Develop and implement treatment plans for patients suffering with mental health concerns
  • Oversee patient registry, which includes tracking patient engagement and behavioral health assessment score(s), creating a baseline and then comparing previous score(s) to determine progress
  • Monitor patient progress and adjust patient treatment plans as needed
  • Participate in regularly scheduled caseload consultations with CoCM team members to review and discuss patient progress and engagement, making sure everyone is on the same page moving forward
  • Facilitate treatment plan changes for patients not improving on their current plans and who may need more intensive or specialized mental health care
  • Maintain accurate and up-to-date documentation in EHR of patient care in accordance with HIPAA regulations
  • Utilize evidence-based practices to support patient progress
  • Coordinate referrals as needed
  • Perform other duties and responsibilities as required, assigned, or requested

Skills & Qualifications:

  • Bachelor’s or Master’s degree in Behavioral Health or related field
  • Valid licensure or certification in Behavioral Health (e.g. LCSW)
  • Experience working in a CoCM is a plus
  • Understand HIPAA requirements for mental health records and maintains appropriate documentation in compliance with legal and ethical standards
  • Experience with documentation in EHRs and knowledge of behavioral health billing codes
  • Strong organizational skills to manage multiple cases and prioritize tasks effectively
  • Ability to make independent decisions while working within a collaborative environment
  • Excellent communication skills
  • Bilingual is a plus
